Trial Chambers pay out through their vaults: a regular vault opens with a Trial Key, an ominous vault with an Ominous Trial Key, and each gives several items at once. How a vault pays out
Each opening draws from three pools. First, one item from the vault's rare pool, or from its common pool one time in five. Then one to three more items from the common pool. Finally a chance at the unique pool: 25% for a regular vault, 75% for an ominous one. Each vault opens once per player.
Trial Keys come from beating trial spawners. Enter the chambers with the Trial Omen, which you get by drinking an Ominous Bottle before going in, and the spawners turn ominous: when beaten, they eject an Ominous Trial Key three times out of ten and supplies the rest of the time.
